1. Full Name
Write your full name in block letters as
recorded in class 10th certificate. If your name has more than 30
characters, abbreviate your middle name suitably and leave a blank
space between initials or part of the name. For example, if your name
is Korasala Venkata Satya Ramchandra Ayer, you may write it as:

2. Date of Birth
Write your date of birth as recorded in your
birth certificate or class 10th certificate. For example if you were
born on 3rd August 1980, fill in as:
3. Category: General (GN), Scheduled Caste (SC), and Scheduled Tribe (ST)
Tick mark the appropriate category you belong
to. For SC and ST category, applicants should support their claim by
submitting a recently attested copy of the caste certificate issued by
a competent authority empowered to issue SC/ST certificate
The authorities empowered to issue scheduled caste / scheduled tribe certificate are:
District Magistrate, Additional District
Magistrate, Collector, Deputy Collector, Deputy Commissioner,
Additional Deputy Commissioner, First class Stipendiary Magistrate,
City Magistrate, Sub Divisional Magistrate, Taluk Magistrate, Executive
Magistrate, Extra Assistant Commissioner, Chief Presidency Magistrate,
Additional Chief Presidency Magistrate, Presidency Magistrate, Revenue
Officer not below the rank of Tahsildar, Sub- Divisional Officer of the
area where the candidate and / or his family normally resides, and
Administrator / Secretary to the Administrator / Development Officer
(Lakshadweep Islands).
Certificate signed by any other official will not be accepted.

Candidates with disability must support their

8. Applicant's Contact Information
Enter your phone number (mobile number / STD code and landline number) and email address in the appropriate places.
9. Qualifying Degree / Diploma Information
Tick mark the appropriate box.
10 Date / Expected Date of Passing
Enter the date of passing of your degree /
diploma. If you are still a final year student, enter the expected date
of passing. For example if you are expecting to pass in June 2007,
write:

Write the name and city of your college /
institute in the box. Write contact details like STD code, Telephone,
Fax number, website, and email address of the college / institute.
12. Enclosures
Please check all your enclosures as listed and
tick in the appropriate boxes. Candidates who have downloaded the form
should fill the details of the enclosed Demand Draft.
13. Declaration
Read the statement and put your signature and
date at the proper place. Signature in the form of upper case letters
(capital letters) will not be considered as a valid signature.
14. Mailing address
Enter your name and postal address in capital
letters. Use black ink only, as this address will be scanned and
transferred to Admit Card and Score Card. The postal address should be
valid till receipt of the score card.
15. Photograph
Paste a good contrast photograph within the
box. Do not staple as the photograph will be scanned. Do not sign on
the photograph. Do not get it attested.
16. Signature
Put your signature, identical to the signature in item 13, in black ink within the box.
Acknowledgment Card / Acknowledgment Slip
If you have purchased the brochure and the
application form, by paying the application fee through a Demand Draft,
fill your complete postal address on the Acknowledgement Card which
will be mailed to you. No stamp has to be affixed on the card.
If you have downloaded the application form,
fill the Acknowledgement Slip by filling name and the details asked for
in the space provided. Leave the space for Application Number blank, as
it will be assigned by the GATE Office.

Choice of Examination Centre
Application No. and examination centre should be quoted in all further enquiries.
Application Submission
If you have purchased the brochure and the
application form by paying the application fee through a Demand Draft,
submit your application in the envelope provided. Otherwise use a good
quality envelope not smaller than 140 mm ? 215 mm. The application
should not be folded lengthwise.
The application may be submitted in person at the counter of GATE Office, IIT Bombay or mailed to
GATE Office
